     "PLEASE DO NOT SUBMIT THIS FORM TO VERIZON PRIOR TO THE FIRST

DAY OF REPORTED ABSENCE.  IT WILL NOT BE ACCEPTED BY VERIZON UNTIL

AFTER THE FIRST DAY OF REPORTED ABSENCE."

Form to Appeal FMLA Denials for Lack of Hours

Use this form if you have been denied FMLA by ARC for not having met the

1250 hour rule even though certified for a chronic condition.  It should be

returned to ARC via the Fax # on the form with supporting documentation.
